Ankle pain can have a range of causes, affecting the bones, ligaments, tendons or surrounding tissues. It may develop suddenly following an injury or gradually over time, depending on the underlying issue.

Persistent or severe ankle pain should always be assessed by a professional. Initial self-care may include rest, ice, compression and elevation (RICE), but if symptoms do not improve or continue to affect your mobility, further assessment is important.

Common causes of ankle pain include:
  • Sprains and strains: Sprains involve injury to the ligaments that support the ankle, while strains affect the surrounding muscles or tendons. These can occur suddenly or build up with repeated stress.
     

  • Bone-related issues: Conditions such as stress fractures or extra (accessory) bones can sometimes affect ankle movement and stability.
     

  • Arthritis: Osteoarthritis, often linked to wear and tear, and rheumatoid arthritis, an autoimmune condition, can both cause pain, stiffness and swelling in the ankle joint.
     

  • Bursitis: Inflammation of the small fluid-filled sacs that cushion the ankle can lead to localised pain and swelling.
     

  • Tendon problems: Tendons around the ankle, including the Achilles, peroneal and tibial tendons, may become inflamed through overuse or injury, causing pain and stiffness.
